Sometimes we all need a little extra help with our follow-through; and four ears can be better than two. Youth are supported in accessing and navigating community and medical services. Community Health Workers (CHW) accompany youth to health, social services, and other important appointments and also provide referrals to trusted providers for physical and mental health needs. By establishing trusting relationships with youth and their families, CHW provide social support, informal counseling, and advocacy for individual and community health needs.
Eligibility: Age 15-24, living with an HIV diagnosis, Mental Health diagnosis, or Substance Use disorder
